Public Health Nursing/Home Visit Services 1270 Natividad Road Home Visitation Programs include Public Health Nursing, ACT (serving pregnant and parenting teens to 18 years), Cal-Learn (pregnant and parenting teens on Welfare), and the First Time Mothers Program, and Lead Poisoning Prevention. Families visited receive assessment, education and support including referral to needed services. Visits focus on working families to build strengths and self-sufficiency, and to prevent long term problems. Most visits are free or on a sliding scale. Referrals can be made by calling or faxing to the numbers above.
