NURSES AND DIETICIANS OF THE MONTEREY COUNTY REGIONAL TEAM

The Monterey County Regional Team consists of Public Health Nurses (RN, PHN), Licensed Vocational Nurse (LVN), and Registered Dieticians (RD). The nurses and dieticians work together to assess the individual, the home, and the community in order to help overcome a client’s health barriers. The needs of the clients is a priority. The nurses and dieticians work hard to provide clients with a basic understanding of the services in the community as well as basic health information.

MAKING A REFERRAL TO THE AB109 TEAM:

If a client presents to Access who you think may be eligible for AB109 services:
  • Call the AB109 Behavioral Health team (796-1248 or 796-1232) to find out the probation status
  • If the client is eligible for AB109 services then coordinate with them to transfer the case to AB109.
