Consumer Perception Survey Data Collection Fall 2019 (November 4-8) The California Institute for Behavioral Health Solution (CIBHS) is currently coordinating data collection for the California Consumer Perception Survey. The resulting information is used to evaluate programs and to fine-tune services to ensure the best outcomes for consumers and families. Monterey County Behavioral Health Survey dates are November 4-8, 2018, we have attached the PDF’s for your use with County Code and 0’s in the bottom of each page where County Client Number goes. You will notice we have added an additional short-survey (Monterey County Adjunct Perception Survey) in English and Spanish.

This adjunct survey should NOT replace the California Consumer Perception Survey required by the State. Our aim is to utilize the data collected from these surveys to address consumer needs as quickly as possible. Please return these surveys along with the State issued surveys.

We appreciate the feedback of our consumers, and your help in ensuring data be collected. We appreciate your support in helping collect this feedback.
