The County provides you with a Group Term Life and Accidental Death and Dismemberment insurance coverage through MetLife Insurance Company. Your coverage begins the 1st of the month following your hire date. For example, if you are hired on March 20th your coverage is effective April 1st. Your coverage amount is based on your bargaining unit. You are not able to increase the coverage amount of your policy. If you wish to purchase more life insurance at your own expense you can purchase an optional policy through AFLAC.
Your policy ends when your employment with the County terminates however, you can generally convert your policy into an Individual Whole Life policy upon your separation by contacting MetLife.
Complete the Beneficiary Designation Form and mail it to MetLife directly as indicated on the form. The County does not keep beneficiary designation forms on file.
